The weight-loss shot is becoming a pill, and the number that matters is not on the scale. The pill is not a better drug. It is a more shippable one.
The oral small molecules land a step below the injectables: orforglipron took off 11.2 percent in phase 3, against 14.9 for the semaglutide shot and 20.9 for tirzepatide. What changes is the factory. A small molecule needs no cold chain and goes wherever aspirin goes, so a health system without refrigeration could actually buy it.
The catch never makes the headline: at the highest orforglipron dose, about one in three had nausea, one in four vomited, one in ten quit. Only oral semaglutide is approved as I write this; the rest are still investigational.
